Summative Assessment Example: Case Formulation Assignment

Students will be assigned a case study and corresponding self-reported scores on screening measures commonly employed in integrated behavioral health settings. Using this information, they will complete the following tasks:  

  • Score the handwritten responses on the PHQ-2, PHQ-9, GAD-7, and C-SSRS screening measures. Then, interpret the screening measure score (i.e., what is the estimated severity or risk level?) 
  • Combine the case study information and results on the screening measures, tease out biological, psychological, and social factors that influence the concerns that the patient is presenting with and place these into respective predisposing, precipitating, perpetuating, and protective factors using the 4-P Model chart below. 
  • Write a concise biopsychosocial formulation of the case study patient.  
  • Propose two appropriate next steps in patient care. This could include recommendations for care team member engagement, a symptom monitoring plan, or specific interventions with evidence-based justification. Students should provide their rationale for next steps in care and cite their references. 
  • What additional patient information would you have wanted to gather?  
  • How might cultural or contextual factors influence your approach? 

The 4-P factor model is a common tool in whole person care to understand the multiple factors impacting a patient’s presenting condition.  This assignment is designed to confirm familiarity with the building blocks of biopsychosocial assessment including the use of screening measures.  Students score behavioral health screening measures, add the data to table with additional scores completed for hemoglobin AC1C commonly referred to as “A1C” and blood pressure or “BP”.  The student then uses the table to consider priorities with the small sample of patients. The student then proceeds to write a biopsychosocial formulation of each case using the 4-P model, then identifies two possible interventions that may address the concerns for each patient incorporating 1:1 care, team-based care, and community resource referral.
