Summative Assessment Example: Case Study
Assignment Instructions:
Choose a case study scenario. Review the story of a client or patient receiving integrated care and then respond to the prompts. Written responses ought to include two to three professional references per question. The approximate length of this assignment is 3-4 pages excluding the reference page.
Guidance for Instructor:
This summative assessment evaluates students’ ability to apply principles of team-based care to a scenario related to integrated behavioral health. The assignment will assess knowledge of interdisciplinary collaboration, communication strategies, and the application of integrated care models in real-world settings. The assignment may consist of written or oral responses to a case study, to promote critical thinking and practical application of team-based care concepts. This assignment may be modified for dyads or triads.

Sample Cases
Case 1 Behavioral Health in Primary Care Settings
Clara has been a patient of metropolitan primary care for ten years. She visits sporadically and tends to miss annual wellness exams. The administrative assistant who greets the patient provided Clara with screening assessments including the PHQ-9, GAD-7, and AUDIT. The administrative assistant handed the screening tools to the patient’s Medical Assistant (MA) who escorted Clara to a clinic room and proceeded to take vitals including temperature, blood pressure, and pulse. The results are tabulated below. The physician entered the room and asked Clara what she hoped to gain from the primary care visit. Clara mentioned she continues to be under a lot of stress taking care of both her mother-in-law with early onset dementia and her own teenagers. She hoped the doctor would prescribe her a medication for sleep and anxiety. The physician discussed her stress and anxiety in the context of environmental factors and asked Clara if she would visit with the Behavioral Health Support Specialist immediately after the medical visit. Clara agreed. The BHSS had met with Clara in the past when Clara first learned that her mother-in-law would be moving in with her.

Case 2 Primary Care in Behavioral Health Settings
Jason is a patient of Union Behavioral Health, a not for profit, county funded licensed behavioral health agency that provides primary care services through a contract with a local primary care provider. The agency provides mental and behavioral services on the third floor of the office building. One room on the second floor serves as the primary care clinic two times per week. Jason has lived with a diagnosis of schizoaffective disorder for fifteen years receiving treatment from the same clinic. Primary care was introduced five years ago. The PCP has changed every six months, so Jason has never seen the same person more than once for primary care services. On the other hand, Jason has been working with the same medical assistant and nurse for two years. Additionally, the psychiatrist and case manager in the behavioral health agency have provided care to Jason for five years.
